Never worked in the oil industry but I’ve worked in manufacturing for most of my adult life so far and I can confirm the equipment in use is kept in piss-poor condition, barely maintained, and often several decades out of date. This has been true everywhere I’ve worked and even the places I’ve toured but didn’t get hired for. It is fairly consistent across all U.S. industry that our facilities are left in decrepit states with machinery that barely functions so I wouldn’t be surprised at all if it really was just an accident and something just broke down due to poor maintenance.
